The process is intended to begin with an initial conversation. Families or referring partners may reach out to discuss an individual's general situation, helping determine whether an assessment may be an appropriate next step.
2. Comprehensive Assessment
Prior to any decisions, a thorough assessment may be considered to better understand care needs, mobility considerations, and personal preferences, as well as overall compatibility with the environment being developed.
3. Review & Transition Planning
Any potential placement would be based on assessment outcomes and overall compatibility. If alignment is identified, a transition approach may be outlined to support a smooth and respectful move.
